Photo Consult

Unscheduled wound consultation by submitting photographs via HIPAA compliant software,
accessed through our secure inForm Care portal.

Video Consult

Scheduled & conducted via HIPAA compliant software, accessed through our secure inForm Care portal.

Step 1
Sign-in to the secure, HIPAA compliant inForm Care portal.
Step 2
Schedule a video call through the portal.
Link to join the video call will be availble in your portal calendar.
Step 3
At least two hours before the scheduled video call, we require the submission of a Wound & Ostomy Consult Request Form to provide the needed patient data to our team.
Step 4
Once the treatment recommendations document is read & implemented, your team will mark the task as "complete" & the status of that task will change from "open" to "complete".
