Implement skin care protocol to prevent:

Incontinence Associated Dermatitis (IAD)

Intertriginous Dermatitis (ITD)

Staff will be educated on the protocol & will receive the needed support to carry it out

Ensure wound care protocols are updated and in compliance with NPIAP criteria

Implement quarterly prevalence study to assess the occurrence of hospital acquired pressure injuries (HAPI)

Educate nursing staff on correct use of Braden Scale

Educate staff on pressure injury preventive measures (PIPM)

Educate staff on identifying moisture skin damage.

Educate staff on how to conduct a prevalence study
